Well, fluid in the lung and massive amounts of sputum along with stabbing chest discomfort lend themselves to pneumonia. But he could have just googled the symptoms and all that. Besides, if he goes to a doctor and they think he has pnuemonia, he gets to go to the hospital anyway to get a chest x-ray to confirm, and all he did was add an extra step.
Not all ERs are super busy like the ER in the television program ER. The last time I went to one - for shooting chest and leg pain, which I thought might be a DVT but turned out to be nothing - there was no one in the waiting room at all. This was in the sticks, in Missouri, so I can't imagine wherever the hell he is being much busier. Especially this early in the morning.
